Section 1.01: Applicant acknowledged that NO mail will be released to him or her until this application and the USPS PS Form 1583 have returned to our office within 15 days from the day the account was opened.
Section 1.02: Applicant acknowledged that all document must notarized, such as; a copy of the USPS form 1583 listed below along with two (2) notarized color photocopies of acceptable ID returned to our office within 20 days from the day the account was opened.
Section 1.03: Applicant acknowledged that the notarized document cannot be dated no more than ten (10) days before or within twenty (20) from the day the account was opened.
Section 1.04: Applicant acknowledged that the mailbox account will not be activated without all completed document.
Section 1.05: By completing this application and USPS Form 1583, applicant appointed TripleMs as agent for the receipt of mail for a period not to exceed for which rent has been paid in advance.
Section 1.06: Applicant acknowledged that a copy of the USPS PS Form 1583 will be made available to the United States Postal Service
Section 1.07: TripleMs provides a fillable PDF format of the USPS PS Form 1583 with instructions.
Section 1.08: Applicant will pick up mail at least twice a month or make other suitable arrangements with TripleMs.
Section 1.09:Applicant understands that the relationship of the parties hereto is one of bailment & not landlord and tenant.
Section 1.10: Applicant will be charged a $15.00 fee for mail pickup, if applicant is registered as a “Mail Forwarding Only Account” client, and has not signed up for local box rental service for the same mailbox.
Section 1.11: Once M&M Mail Services representative has placed the applicant’s mail in the assigned lock-box, the mail shall be deemed to have been delivered.
Section 1.12:M&M Mail Services shall not be responsible for loss, theft or damage of mail or packages done by the Post Office USPS and any other carriers.
Section 1.13: M&M Mail Services is not engaged in the delivery of mail and cannot be responsible for failure of the United States Postal Service to deliver mail or to deliver it in a timely fashion or undamaged condition.
Section 1.14: Applicant agrees to use services in accordance with M&M Mail Services rules and in compliance with all U.S. Postal regulations, as well as local, state and federal statutes and regulations. Failure to do so may result in cancellation of service without notice. NO REFUND will be provided.
Section 1.15: Information provided by applicant will be kept confidential and will not knowingly be disclosed without applicant’s prior consent, except for law enforcement or postal operation purposes, in which case M&M Mail Services intends to cooperate fully.
Section 1.16: Law enforcement is further clarified to include all cities, counties, states or federal agencies or their representatives.
Section 1.17: Applicant acknowledged that all free riders must be minor relatives residing with the applicant.
Section 1.18: Applicant further agrees that parcels delivered to this address will be delivered by common carriers only, that no truck line deliveries will be made, that parcels will be retrieved within 48 hours after delivery, and that no hazardous or dangerous material will be delivered to applicant. Failure to adhere to any of these parcel delivery stipulations will result in termination of service.
Section 1.19: Applicant agrees to protect, indemnify and hold harmless M&M Mail Services from and against any and all claims, demands and causes of action any nature whatsoever relative to use of M&M Mail Services facilities or services.
Section 1.20: Should TripleMs commit or fail to commit any act that results in disruption of service and applicant thereby suffers a loss, TripleMs’ liability shall be limited to not more than the rental fees paid by applicant for service not yet received.
Section 1.21: M&M Mail Services shall not be liable for incidental or consequential damages.
Section 1.22: Per USPS regulations, certified, registered, insured, or C.O.D. mail or parcels will be accepted by M&M Mail Services on behalf of applicant. Full, advance payment of C.O.D. charges must be made to M&M Mail Services (cash only) 48 hours prior to acceptance of any C.O.D. packages.
Section 1.23: TripleMs fees are due and payable in advance and notice thereof may be placed in applicant’s lock-box, email inbox or text mail. No other notice will be required. Failure to pay such fees when due may result in disruption or cancellation of services.
Section 1.24: M&M Mail Services does not prorate fees and does not provide refunds in the event of cancellation by applicant.
Section 1.25: Applicant shall use only the address designation “PMB” or “#” to designate their address. NO OTHER DESIGNATION IS VALID. Specifically excluded is the use of suite, apt., dept., or other designators.
Section 1.26: Upon termination of services by M&M Mail Services or failure to pay rent in advance by applicant, M&M Mail Services shall not make applicant’s mail available without payment theretofore.
Section 1.27: Applicant understands that the United States Postal Service will not forward or return mail without payment, and will not accept a Change of Address from applicant.
Section 1.28: Applicant is responsible for notifying each correspondents of their new address upon cancellation of lock-box account.
Section 1.29: At termination of service, applicant, if he wishes mail forwarded after that date, shall provide to M&M Mail Services with a forwarding address and pay the required fees.
Section 1.30: In the event applicant fails to provide the forwarding address and fee, TripleMs shall refuse any further mail, in the case of mail already received, TripleMs will handle such mail in accordance with USPS DMM D042.2.6 regulations.
Section 2.01: Key loss incurs a $28 replacement charge, applicant agrees to immediately notify TripleMs in event of loss of key.
Section 2.02 If you have an Access Security Card, the cost to replace it is $35 per card.
Section 2.03 The minimum forwarding deposit on active mailbox is $50.
Section 2.04 Mail forwarding fee is $3.95 per forwarding action on an active mailbox, plus postage and packaging charges.
Section 2.05 Termination of service, if applicant wishes to have mail forwarded shall provide a forwarding address and pay a $100 forwarding deposit required fees.
Section 2.06 Mail forwarding fee is $5.95 per forwarding action on a terminated mailbox, plus postage and packaging charges.
Section 2.07 TripleMs shall refuse any further mail if applicant fails to provide a forwarding address and the required fees.Section 2.08 The key deposit shall be refunded upon return of the key within thirty business days of termination of service.
a) Exception: All extra or additional keys and/or cards are non-refundable.
Section 3.01 Fees are due and payable in advance and notice thereof will be placed in applicant’s lock-box. No other notice will be required. Failure to pay such fees when it’s due may result in disruption, hold of mail or cancellation of service.
Section 3.02 TripleMs may hold mail if the mailbox rental installment or any other fees associated to it have not been paid.
Section 3.03 TripleMs doesn’t prorate fee or provide any refunds in the event of cancellation by applicant.
Section 4.01 Package Notifications by Email. Not Guarantee.
Section 4.02 Package Notifications by Phone Calls, Not Guarantee.
Section 4.03 Package Notifications by Text Messages. Recommended and 99% Guarantee.
Section 5.01 Applicant shall use only the address designation, “PMB” or “#” to designate their address. NO OTHER DESIGNATION IS VALID. Your package may result in delivery delay or non-delivery of mail if any other designation address is used. When addressing your address, exclude the use of suite, apt., dept., or similar designators.
Section 5.02 The U.S. Postal Service may refuse to deliver any piece of mail that does not include the PMB or # sign designation.
Section 5.03 The address to be used by applicant for the purpose of receiving mail is as follows:

Section 6.01 Upon termination of services by TripleMs or failure to pay rent in advance by applicant, TripleMs shall not make applicant’s mail available without payment theretofore. Applicant understands that the United States Postal service will not forward or return mail without proper stamp amount, and will not accept a “Change of Address” from neither the private mailbox account holder nor the private mailbox companies.
Section 6.02 At termination of service, applicant, if he wishes mail forwarded after that date, shall provide to TripleMs with a forwarding address and pay the required fees.
Section 6.03 In the event applicant fails to provide the forwarding address and the applicable fees, TripleMs shall refuse any further mail and, in the case of mail already received, TripleMs shall handle such packages or mail in accordance with USPS DMM regulations.
Section 6.04 Customer further agrees that TripleMs may terminate or cancel this Mail Receiving Service Agreement for good cause at any time providing our customer with a thirty (30) day written notice.
(a) Good cause shall include, but is not limited to:
1) Abandonment
2) Use for criminal, illegal or illegitimate activity
3) Failure to pay money owed to TripleMs
4) Use of mail receiving inconsistent with this Agreement,
5) including receipt of unreasonable volume of mail
6) Non-performance of the terms of this Mail Receiving Service Agreement
7) In violation of the USPS DMM Regulations.
Section 6.05 TripleMs, at its sole option and discretion, may terminate applicant’s mailbox service immediately without any advance notice, if TripleMs is determined that applicant’s behavior is unsafe or contrary, and to an accepted standard behavior or to the prudent conduct of business.
Section 6.06 At termination of service, I hereby instruct M&M Mail Services as follows:
Section 6.07 Forward my mail to my new address for up to 6 months on;
1) Weekly basis, or
2) Monthly basis. After that time frame, incoming mail will be destroyed and/or returned to sender.
Section 6.08 Mail forwarding fees are $5.95 per forwarding action on terminated mailbox, plus postage and packaging charges.
Section 6.09 Forwarding address must be provided in writing, at time of termination, or mail will not be forwarded.
Section 6.10 I place $_______________ on deposit, at time of mailbox rental, for the purpose of mail forwarding, after contract expiration or cancellation.
Section 6.11 Do not forward my mail. I understand that mail will not be forwarded to me, and will be destroyed and/or returned to USPS accordance with the USPS DMM D042.2.6 Regulations.
Section 6.12 The U.S. Postal Service will not accept an address change form from your Private Mailbox to your new address. This is the mailbox holder’s responsibility to inform the new address to his/her contacts.
Section 7.01 The notary signature must performed if;
a) The applicant is not physically present, or
b) The applicant is an out-of-state applicant, and/or
c) The applicant has an out-of-state license, and/or
d) The applicant has presented a foreign passport.
Section 7.02 Both, the USPS PS Form 1583 and the two colored photocopies must be signed, dated and notarized.
Section 7.03 If all document presented in person, NO NOTARY SIGNATURE IS REQUIRED.

Section 8.01 Your responsibility is your top priority. Means that; your verbal conversation with our representatives must be very respectful and fully professional.
Section 8.02 TripleMs will provide a lock-box key or combination (applies to local walk-in clients only) to applicant who may obtain his/her mail during the business hours posted by TripleMs.
Section 8.03 Should applicant appointed another person or organization, TripleMs shall assume that the possession of a key is evidence of authority to collect mail.
Section 8.04 Key(s) must be returned upon termination of service. Applicant understands that the relationship of the parties hereto is one of bailment and not landlord and tenant.
Section 8.05 The key loaned to applicant (applies to local walk-in clients only) shall require a refundable cash deposit, and remains the property of TripleMs and shall not be duplicated or modified by applicant.
Section 8.06 The key deposit shall be refunded upon return of the key within thirty business days of conclusion of service.
a) Exception: The lock-box keys and any extra keys hold by Applicant will not be refunded for any reasons.
Section 9.01 In a small-sized mailbox: For up to two (2) persons, five (5) free riders and/or one (1) organization in a single lock-box and each must complete a USPS Form 1583 and provide photo identification and proof of corporation.
Section 9.02 In a medium-sized mailbox: For up to five (5) persons and/or one (1) organization in a single lock-box and each must complete a USPS Form 1583 and provide photo identification.
Section 9.03 In a large-sized mailbox: For up to seven (7) persons and/or organizations in a single lock-box and each must complete a USPS Form 1583 and provide photo identification.
Section 9.04 In any-sized mailbox: For up to twenty-five (25) persons and/or organizations in a single lock-box and each must complete a USPS Form 1583 and provide photo identification. (This only apply to advertising and statutory accounts.).
Section 9.05 If applicant consistently receives substantially more mail than can be placed in a single lock-box, TripleMs reserves the right to require the applicant to rent a larger size mailbox or one or more additional boxes or charge an additional fee.
Section 9.06 Charges for service are based upon average daily volume and activity. Special circumstances, e.g., high number of parcels, etc. may require assessment of additional fees. An unusually high volume of mail will result in either a higher fee being charged, or termination of the mail receiving service.
Section 10.01 At termination of service, I hereby instruct M&M Mail Services as to do one of the following:
a) ? Forward my mail to my new address listed below. In consideration thereof, I place $ ________.____ on deposit to be used for this purpose.
b) ? Do not forward my mail. I understand that mail will not be forwarded and may be disposed and/or returned to senders.
Section 11.01 Renewal of this Mail Receiving Service Agreement for additional terms shall be at TripleMs’ sole discretion.
Section 11.02 As customer’s authorized agent for receipt of mail, M&M Mail Services will accept all mail, including;
(a) Registered mails
(b) Certified mails or documents,
(c) Packages delivered by UPS, USPS, DHL or FedEx Express and/or Ground, and Packages delivered by private carrier.
Section 11.03 In the event customer refuses to accept any such mail, customer shall pay postage and other fees associated with refusal and return of such mail.
Section 11.04 Any C.O.D. items will not be accepted.
(a) Unless the Customer has a current update credit card on file,
(b) Also agrees to pay a $10 service fee and 7.95% interest rate on any credit card transaction.
(c) The C.O.D. must not exceed $1500 in total maximum of three (3) per month.
Section 11.05 Customer further agrees to protect, indemnify and save and hold harmless Truckalogy, Inc. and all of its trade marks and names from and against any and all claims, demands, and cause of action arising out of or in connection with the use or possession of a mail receiving, including without limitation, any demands, claims and causes of action for personal injury or property damage arising from such use or possession;
(a) From failure of the U.S. Postal Service (USPS) and any other carriers to deliver on time or otherwise any items (mail, parcels, package, etc.);
(b) From damage to or loss of mail receiving contents by any cause whatsoever and
(c) From any violation by customer of applicable federal, state or local laws.
Section 11.06 Customer herein agrees that the total liability of M&M Mail Services for any and all claims is limited to $55.00 regardless of the nature of the claim _________ (initial).
Section 11.07 The Use of the recommended format above will assure you of the earliest possible delivery from the U.S. Postal Service and any other carriers.
Section 11.08 For delivery by common carriers, such as USPS, UPS, Federal Express, etc., please include the box number, it’s a must.
(a) Please Note That, Items Are Delivered By Box Number. Articles That Have Insufficient Addressing May Be Returned To Sender Or Disposed.
Section 11.09 Billing: automatic billing payment may assess a percentage up to 7% recurring charge. Online payment is available at TripleMs.com/
(a) Other method of payment: visa payment may also assess a percentage charge of up to 5% on Visa and MasterCard and up to 10% on American Express.
(b) Check Payment: Please make checks payable to M&M Mail Services.
Section 12.01 24/7 access to your mailbox deposit is required: $15.00/key or $35/card
Section 12.02 $35.00 Refundable security deposit required. To receive your refundable deposit you must first;
1) Cancel your account with us, minimum thirty (30) days advance notice is required.
2) Your mailbox account must be in good standard. All obligations must be paid.
3) All of your mail must be redirected to your new designated address within thirty (30) days.
4) After thirty (30) days, M&M Mail Services reserved the right to collect the monthly installment fee from the refundable deposit and/or hold such deposit for up to ninety (90) days to assure that the Customer honored this agreement.
5) We must not receive any mail to your mailbox for you or your subs during that ninety (90) days period from the date the mailbox holder was requested a cancellation on the mailbox account.
Section 12.03 If mails are still receiving on your behalf, or for your add-on names, an email notice will send to you as a reminder to reroute or forward the mails to your new address.
(i) Email notification service fee is $5 flat rate per month on a cancelled account.
(ii) $10.00 non-refundable application fee,
(iii) $15 Notary and service fee for the USPS form 1583 (Applicable for out of state or by mail applicants).
a) FORM 1583 IS REQUIRED BY UNITED STATES POSTAL SERVICE (USPS). A $15 NOTARY AND SERVICE FEE WILL APPLY IF FORM 1583 RETURNS WITHOUT NOTARIZED.
Section 12.04 TripleMs shall use its best efforts to give customer possession as nearly as possible at the beginning of the rental term. If TripleMs is unable to timely provide the Rental Premises, rent shall abate for the period of delay and customer shall make no other claim against TripleMs for any such delay.
Section 12.05 Customer may renew the rent on a monthly basis. Customer shall exercise such renewal option, if at all; by giving written notice to TripleMs not less than ninety (90) days prior to the expiration of the Initial Term. The renewal term shall be at the rental set forth above or else upon the same covenants, conditions & provisions as provided in this rental.
Section 13.01 Customer shall pay to TripleMs during the Initial Term rental of each installment payment. Installment shall be due in advance, on the first day of each calendar month during the rental term to TripleMs or M&M Mail Services at 2515 E. Thomas Rd., Ste. 16 Phoenix, AZ 85016 or at such other place designated by written notice from TripleMs to Customer.
Section 13.02 The rental payment amount for any partial calendar months included in the rental term shall be prorated on a daily basis. Customer shall also pay to TripleMs security deposit in the amount of $35.00 which can be arranged to be paid within three months
Section 14.01 Notwithstanding the forgoing, Customer shall not use the Rental Premises for the purposes of storing, manufacturing or selling any explosives, flammables or other inherently dangerous substance, chemical, thing or device.
Section 15.01 Customer shall have the right of TripleMs’ consent, to assign this rent to any corporation or individual with whom Customer may merge or consolidate, to any subsidiary of Customer, to any corporation under common control of Customer, or to a purchaser of substantially all of Customer’s assets.
Section 15.02 The maximum Customer per box are as followed:
(i) Personal: Three (3). $7.50 thereafter.
(ii) Business: One (1) business name and three (3) individuals $10.50 thereafter.
(iii) Corporation: One (1) business name and five (5) individuals. $15.50 thereafter.
Section 15.03 Except as set forth above, Customer shall not sub rent all or any part of the Rental Premises, or assign this Rental in whole or in part without TripleMs’ consent, such consent not to be unreasonably withheld or delayed.
Note: Form 1583 Is Required By United States Postal Service (USPS). A $15 Notary And Service Charge Will Apply To The Account If Form USPS 1583 Returns Without Been Notarized.
Section 16.01 On behalf of the State and authorities, TripleMs shall collect from customer the sales and/or the property taxes.
Section 17.01 TripleMs does not provide any personal property or liability insurance to customer.
Section 17.02 If the Rental Premises is damaged by fire or other casualty resulting from any act or negligence from customer or any of customer’s agents, or one of your employees, rent shall not be diminished or abated while such damages are under repair, and customer shall be responsible for the costs of repair not covered by insurance.
Section 17.03 TripleMs may keep fire and extended coverage insurance on the rental premises in such amounts as TripleMs shall deem appropriate.
Section 17.04 Customer shall be responsible, at its expense, for fire and extended coverage insurance on all mails and packages located at the rental premises.
Section 18.01 The Security Deposit shall be held by TripleMs without liability for interest and, it’s being expressly understood that the Security Deposit shall not be considered an advance payment of rental or a measure of TripleMs damages in case of default by Customer.
Section 18.02 Unless otherwise provided by mandatory non-waivable law or regulation, TripleMs may commingle the Security Deposit with its other funds.
Section 18.03 TripleMs may, from time to time, without prejudice to any other remedy, use the Security Deposit to the extent necessary to make good any arrearages of rent or to satisfy any other covenant or obligation of customer hereunder.
Section 18.04 Following any such application of the Security Deposit, customer shall pay to TripleMs on demand the amount so applied in order to restore the Security Deposit to its original amount.
Section 18.05 If customer is not in default at the termination of this Rent, the balance of the Security Deposit remaining after any such application shall be returned by TripleMs to Customer.
Section 19.01 Customer shall comply with all USPS and US state and federal laws, orders, ordinances and other public requirements now or hereafter pertaining to customer’s use of the rental premises.
Section 19.02 TripleMs shall comply with all USPS and US state and federal laws, orders, ordinances and other public requirements now or hereafter affecting the rental premises.
Section 19.03 This Mail Box Rental Agreement terminates and supersedes all prior understandings or agreements on the subject matter hereof.
Section 19.04 This Agreement may be modified only by a further writing that is duly executed by TripleMs upon rental term renewal.
Section 19.05 TripleMs reserved the rights to change this agreement from time to time without the customer’s knowledge.
